Comments:
Licensing Inspector arrived at 11:30 AM and departed at 1:00 PM to conduct a renewal inspection. At the time of the inspection, there were nine children (ages infant through preschool), for 27 points in care with the provider. Children were observed preparing for and napping. The sample size was nine children's records and the provider's record.
Based on review of nine children's records, it was determined that the provided did not ensure that all required information was present in each child's record.
Evidence: 1. The record for child #1, who was in care at the time of the inspection, did not contain addresses for the two required emergency contact persons. 2. The record for child #8, who was in care at the time of the inspection, did not contain an address for one of the two emergency contact persons and the second emergency contact person was missing.
Plan of Correction:
Provider will contact parents to obtain updated information and place it in the records.

Description:
Based on observation and review of children's records, it was determined that the provider did not ensure that a caregiver does not exceed 16 points.
Evidence: At the time of the inspection, there were nine children in care for a total of 27 points with only the provider.

Provider will ensure that an assistant is present whenever there are more that 16 points in care.
