Comments:
An unannounced monitoring inspection was conducted today from 10:30am-11:30am. There were 5 children (20 points) directly supervised by 2 staff. The home, 2 staff records, 1 child?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. There was a supply of books, toys, and materials for the children. An area of non-compliance is identified in the Violation Notice.

Based on record review, the provider did not ensure all children's records were complete.
Evidence: Child 1 does not have signed documentation of viewing the original proof of identification
Plan of Correction:
I viewed the original. I will make sure to sign the documentation of viewing the original birth certificate.
