Comments:
An unannounced renewal inspection was conducted today between the hours of 1:50pm and 4:30pm. There were 3 children directly supervised by the provider. The home, general procedures, the health and safety checklist, and background checks were inspected. Children were observed during naptime and playing freely. Areas of non-compliance are identified in the Violation Notice. Based on record review, 2 of 3 children's records reviewed did not contain all required information.
Evidence: 1. The record for Child #1 did not contain the following information: -Name, address and telephone number of each parent or other responsible person or persons; -Name, address and telephone number of each parent's place of employment and his or her work hours; -Name, address and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care; -Name, address and telephone number of the child's physician; -results of a health examination -Any known or suspected allergies and any chronic or recurrent diseases or disabilities;
2. The record for Child #2 did not contain the parent's signed authorization for the child's emergency medical treatment, and results of a health examination.
