Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:20pm. There were 7 children (20 points) directly supervised by 2 staff. The provider has received approval from Fairfax County Zoning Division for a maximum capacity of 12 children. The space designated for use of operation of the family day home was evaluated during the inspection, two levels of the home are currently being used to care for children. An increase in license capacity from 7 children to 12 children will be reviewed. A discussion was held with the provider regarding the need to use both areas, the lower level and the upper level, in order to be able to increase the license capacity. The home, 2 staff records, 5 children?s records, 4 household member's records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, listening to stories, playing with small manipulative toys, and napping. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Evidence: Child#1's record is missing the street name and the street number for 1 of 2 required emergency contacts. Child#2's record is missing a written authorization for emergency medical care. Child#5's record is missing the father's work phone number.
