Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated on September 8, 2021 and concluded on September 9, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 3 months to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of two children's records, one does not contain the required documentation. Evidence: The record for child #1 does not contain documentation of a physical examination prior to first day of attendance.

I will get a copy of their physical and make sure to have one before a child's first day.

Description:
Based on review of two household member records, one does not contain the required documentation of a current tuberculosis (TB) screening. Evidence: The record for household member #1 does not contain documentation of a current TB screening. Current documentation expired in March 2021.
Plan of Correction:
A new TB screening will be completed.
