Comments:
A 60 day focus monitoring inspection was initiated on 6/5/2024 from approximately 11:00 am to 12:00pm. There were a total of 10 children with 3 caregivers supervising. Children were observed during snack time, engaged in group activities, playing with cars and lunch time. The children were served chicken nuggets, beans, rice, apples and banana. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Repeat Violation:
Based on observation a required posted document by the Superintendent was not posted in a conspicuous place on the licensed premises.

Evidence
The last licensing inspection for the Family Day Home was not posted.

Plan of Correction: The last inspection has been posted.

Description: Repeat Violation:
Based on review of three caregiver records, the family day home did not obtain a copy of the results of a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years.
Evidence:
Caregiver #2 has lived outside the state of Virginia within the preceding five years in New York.
There was no documentation of the results of a search of the child abuse and neglect registry from the state of New York.

Plan of Correction: We have completed and received the results of the caregiver?s background check, however it was not accepted. We are working on correcting any mistakes and redoing it.

Description: Based on observation, the provider did not ensure topical skin products were inaccessible to children.
Evidence
Topical skin products and cosmetics were accessible to children in cabinet located under the bathroom sink.

Plan of Correction: We have ensured that all tropical skin products are stored in a closet inaccessible to the children.

Description: Based on review of emergency evacuation procedures, the provider did not have documentation of all monthly emergency evacuation drills.
Evidence
There was no documentation of an emergency evacuation drill for May 2024.

Plan of Correction: We have now documented our latest monthly evacuation drill and will continue to do so in the future right after practiced.

Description: Based on observation the posted menu did not reflect substituted food served.
Evidence
The menu posted did not reflect what was served to the children for lunch.

Plan of Correction: We had served what was on the menu, it said chicken and we served chicken nuggets and just added some extra snacks.

Description: Based on observation, the provider did not ensure children's bottles were refrigerated and labeled.
Evidence
Two bottles located on the kitchen counter were not labeled with child's name and date.

Plan of Correction: We have labeled all children?s bottles and will ensure they?re refrigerated from now on.

Description: Based on observations, the provider did not ensure potentially poisonous substances, materials and supplies were stored away from food in areas inaccessible to children.
Evidence
Clorox bleach and disinfectant wipes were in a low cabinet under the kitchen sink that was accessible to children.

Plan of Correction: We have moved all potentially poisonous substances, materials and supplies out of the daycare area.

Description: Based on observation, the provider did not ensure sharp kitchen utensils and other sharp objects were inaccessible to children.
Evidence
Four knives were located in a drawer in the kitchen that was accessible to children.

Plan of Correction: We will be keeping all sharp objects in a high cabinet inaccessible to children.

Standard #: 8VAC20-800-320-A
Description: Based on observation, the provider did not ensure all small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers, and irons were unplugged when not in use.
Evidence
In the kitchen a tea pot and toaster were plugged in and not being used by the caregiver or with children under close supervision.

Plan of Correction: We have ensured to place all electrical appliances in cabinets and unplugged when not in use and will continue to do so in the future.

Standard #: 8VAC20-800-320-H
Description: Based on observation, the provider did not ensure all flammable and combustible materials were stored in an area inaccessible to children.
Evidence
A lit candle and lighter were located on the kitchen counter that was accessible to children.

Plan of Correction: We will no longer be using candles in the daycare.

Standard #: 8VAC20-800-340-C
Description: Based on observation, the provider did not ensure the bathroom contained toilet paper.
Evidence
The bathroom the children use did not have toilet paper available.

Plan of Correction: We have ensured there is toilet paper in the bathroom and will replace it as soon as it runs out in the future.

Standard #: 8VAC20-800-540-A
Description: Based on observation, the provider did not ensure a safety strap was used and securely fastened while the high chair was in use.
Evidence
Child #1 was not strapped in the high chair during the snack time.

Plan of Correction: Me, my 2 assistants, and daughter don?t recall this happening, would like to appeal this violation.

Standard #: 8VAC20-800-540-B
Description: Based on observation, provider did not ensure Infant walkers were not used.
Evidence
Child #1 was observed being placed in a walker with wheels.

Plan of Correction: We have got rid of the infant walker and will no longer be using walkers.
