Comments:
A monitoring inspection was completed on site on Tuesday, September 13, 2022. The licensing inspector was on site from approximately 10:10am until 11:40am. There were two children present with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and programming. A total of four children?s records, two caregiver records, and one household member?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of two caregivers' records and interview, the vendor did not ensure one caregiver obtained a fingerprint-based national criminal background check prior to employment.
Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain the results of a fingerprint-based national criminal background check. 2) During interview, Caregiver #1 reported the fingerprints for Caregiver #2 have not been requested.
Plan of Correction:
Caregiver #2 will schedule an appointment and submit fingerprints upon receipt.

Description:
Based on a review of two caregivers' records and interview, the provider did not obtain a completed sworn statement for one caregiver prior to the first day of employment.
Evidence: 1) The record for Caregiver #2, date of employment 08/02/22, did not contain a completed sworn statement. 2) During interview, Caregiver #1 reported Caregiver #2 has not completed a sworn statement.
Plan of Correction:
Caregiver #2 will complete the sworn disclosure on 09/14/22.

Description:
Based on a review of two caregivers' records and interview, the provider did not obtain a central registry finding within 30 days of employment.
Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain a central registry finding. 2) During interview, Caregiver #1 reported the central registry search has not been requested.
Plan of Correction:
The central registry request will be completed and sent for processing.

Description:
Based on a review of two children?s records and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.
Evidence: 1) The record for Child #3, date of attendance 09/12/22, did not contain proof of adequate immunization. 2) The record for Child #4, date of attendance 05/09/20, did not contain proof of adequate immunization or a copy of the physical examination results before or within 30 days after enrollment. 3) During interview, Caregiver #1 reported the immunizations for Child #3 and Child #4, and the physical examination for Child #4 have not been obtained. The health and safety checklist requires proof of adequate immunization received on the date of admission, or prior to admission, for each child admitted and a copy of the physical examination results obtained on each child before or within 30 days after enrollment.
4) The record for Caregiver #2, date of employment 08/02/22, did not contain a tuberculosis (TB) test or screening. 5) The record for Household Member #1 had a TB screening that expired on 04/15/2021. The health and safety checklist requires that any adult, including any adult household member, who comes in contact with children or will provide care to children has a tuberculosis (TB) test or screening.
Plan of Correction:
The missing immunizations will be requested and filed. An appointment will be scheduled for the missing TB screening.
