Comments:
An unannounced renewal inspection was initiated and concluded on June 20, 2023 to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from approximately 12:15pm until 1:45pm. The provider assisted the licensing inspector during the inspection. There were two children in care, ranging from 22 months to three years of age with one caregiver present. The licensing inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. The inspector reviewed four children?s records, one provider?s record and one household member?s record.
Information gathered during the inspection determines non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of four children?s records, the provider did not maintain the required information in the record for each child enrolled in care. Evidence: 1. The record for Child #1 (First date of attending 09/18/2019) contained immunization records dated 09/17/2019. Child #1 turned two years of age in the Fall of 2021. 2. The records for Child #2 (first date of attendance 04/11/2022) and Child #3 (first date of attendance 10/17/2022) did not contain the name, address and telephone number of one or more persons designated by the parent or parents to be called in case of emergency. 3. The record for Child #4 (first date of attendance 09/12/2022) contained immunization records dated 05/20/2022. Child #4 turns two years of age in the Summer of 2023. The record did not contain the address of the designated emergency contact.

I will speak with the families, get a copy of the missing information and add it to the records.

Based on review of four children?s records, the provider does not meet all the criteria on the health and safety checklist at the time of the renewal inspection.
Evidence: 1. The record for Child #4 (first date of attendance 09/12/2022) did not contain a copy of physical examination results obtained within 30 days after enrollment.

I will get a copy of the missing physical and add to the record.
