Comments:
The licensing inspector conducted an announced inspection to review staff records at the administrative building on December 13, 2019 from approximately 9:30 AM to 4:00 PM. During the inspection, four staff records were reviewed. Violation of the standards for licensed child day centers were found during the inspection. Based on review of four staff records and interview, one staff did not meet the qualifications for program leader. Evidence: The record for Staff #1 (hire date 2/11/19) did not contain documentation for qualifications for the position of program leader. Program leaders may qualify with six months of supervised programmatic experience if within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 24 hours of training shall be received related to the care of children, including but not limited to child development, playground safety, health/safety issues, and preventing/reporting child abuse and neglect.

Administration will ensure the correct qualifications for program leaders are documented in staff records.

Description:
Based on a review of five staff records, the facility did not ensure that staff received the required training by the end of their first day of employment. Evidence: The licensing inspector observed the record for Staff #2 (hire date 8/13/19) did not contain documentation of the staff having completed the required orientation training.

Administration will ensure that orientation training is completed and documented for each new staff.

Description:
Based on a review of four staff records, the licensee failed to ensure that one staff had a central registry finding within 30 days of employment. Evidence: The record for Staff #3 indicated a hire date of 2/8/17 and there was no central registry finding recorded.
Plan of Correction:
The central registry finding for this staff was misplaced and a new copy has been ordered for this background check. The staff record will be updated as soon as possible.
