Comments:
An unannounced renewal inspection was conducted today from 11:10am-12:50pm. There were 25 children directly supervised by 6 staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing with blocks and puzzle, playing outside, and eating lunch Self care routines and hand-washing procedures were also observed. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Based on records reviewed, child records do not contain all required information. Evidence: Record for Child 1 is missing Parents work place information.
Plan of Correction:
