Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on Thursday, May 14, 2020 and concluded on Friday, May 15, 2020. The director was contacted by telephone to initiate the inspection. There were 11 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Based on review of children's record and interview with staff, the center did not ensure to obtain physical examination within one month after attendance.
Evidence: In Child#2's record (start date:9/16/19), there was no documentation of a physical found. Interview with staff confirmed there was no physical documentation available for review during the inspection.
Plan of Correction:
To make sure upon enrolling new students that all paperwork is complete and all forms are turned in before enrollment.
