Comments:
A monitoring inspection was initiated on January 6, 2023 and concluded on January 10, 2023. There were 27 children present, ranging in ages from infancy to 4 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation and interview, the center did not ensure to follow the staff to children ratios for birth up to 16 months as required.
Evidence: During the inspection, in the Butterflies (Infant) room, there was observation of one staff present with five infants in care. Interview with staff confirmed the ratio for this group is 1:5.
Plan of Correction:
I moved a Child to the toddler room. Ratio is 1:4 now.

Description:
Based on observation, center did not ensure the diapering surface use for only diapering.
Evidence: 1. During the inspection, in the Butterflies room the diapering surface was observed with a face mask stored on top with visible stains on the surface. 2. Photographs of the diapering surface observation during the inspection.
Plan of Correction:
Have spoken to the staff person to let know nothing is to be on changing table and to make sure to keep it clean always.
