Comments:
An unannounced focused monitoring inspection was conducted with the director on May 31, 2024, beginning at 12:04 p.m. and ending at 1:00 pm. Six staff records were reviewed during the inspection. Additional facility documentation was reviewed in compliance.

Based on a review of 26 staff records, it was determined that not all staff records contained a central registry finding. Evidence: Staff A's record did not have documentation of a central registry finding.
