Comments:
An unannounced focused monitoring inspection was conducted with the director on May 9, 2024, beginning at 10:45 a.m. and ending at 12:05 p.m. Three staff records were reviewed during the inspection. An inspection of the physical plant and observations were completed. Additional facility documentation was reviewed in compliance.

REPEAT VIOLATION: Based on a review of documentation and staff interview, it was determined the center failed to have at least one staff member who meets the qualifications of a program leader or program director shall be regularly present for each grouping of children. Evidence: The teacher in the infant room does not possess program leader qualifications.

Based on observation, the center did not ensure that all mattress being used were covered with waterproof material which can be cleaned and sanitized. Evidence: One crib mattress in the infant classroom had scratches and splits making it no longer waterproof. .
