Comments:
An unannounced monitoring inspection was conducted on August 14, 2023, from approximately 10:45 AM until 12:30 PM. There were 80 children present, ranging in ages from 5 months to 11 years, with 12 staff supervising. The children were observed playing on the playground, playing with age-appropriate materials, and handwashing. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on record review and interview, the center did not ensure that at least annually parent confirmation that the required information in the child's record is up to date. Evidence: 1. Child #1's record did not contain an annual update confirming the child's information in the child's record was up to date. Child # 1?s start date was 7/5/2022.
