Comments:
A complaint was received in the licensing office on August 14, 2023 regarding supervision of children. On August 15, 2023 an on-site unannounced investigation was conducted. Four staff were interviewed, and observations were made. Child and staff records, attendance sheets, and incident reports were reviewed. At the time of this investigation, the preponderance of evidence did not support the allegation and the complaint is deemed not valid.
The information gathered during the investigation determined non-compliance with applicable standards or law. Violations, not related to the complaint, were documented on the violation notice issued to the facility.

Based on record review and interview with the director, the licensed child day program did not have a central registry finding within 30 days of employment. Evidence: 1. The record for Staff #2 (DOH 04/04/23) did not have a central registry finding. 2. The record for Staff #3 (DOH 04/28/23) did not have a central registry finding. 3. The director acknowledged neither Staff #2 or Staff #3 did not have a central registry filing in their records.
Plan of Correction:
I will follow up with central registry office today. Going forward, I will be sure to follow-up and document any new searches if I do not receive them within 30 days of employment.
