An unannounced focused inspection was conducted this date. The focus of the inspection was to evaluate the provisions set forth within the
Special Order. Staff training on supervision, use of substitutes and roles and responsibilities were verified. Documents obtained to substantiate
that notification to families regarding an incident in June 2022 had been completed.
