Technical Assistance:
You are reminded that all staff are required to complete the 3 hour refresher training.

One slide on the preschool playground is not age appropriate for preschool age children. As a reminder, only school aged children are permitted on this slide.

Comments:
An unannounced Monitoring inspection was conducted this date. A total of 78 children with 14 staff were present today. Observation of all 9 classrooms was conducted along with both the toddler and preschool playground was made. The center has a chicken coop which was found well maintained and observable from the preschool playground. Several classes were on the playground and basketball court interacting with each other and staff. The children transitioned back to the classrooms, washed hands and sat down for lunch. In the infant classroom, some infants were observed sleeping in cribs, laying on the floor under a mobile while staff interacted with them. The older infants were seated at a table with one staff sitting with them to ensure safe eating. All medication onsite, some topical ointments throughout classrooms and infant daily progress reports were reviewed. A total of 5 children's records and 5 staff records were reviewed. Standard #: 8VAC20-780-270-A
Description: Based on observation of the center, one area of the building was not maintained in a safe condition.
Evidence:
1. In Class #2 on the far right exterior wall, the paint was peeling. This area is accessible to the children in care.

2. Activity tables located on the Toddler playground had standing soiled water with leaves that had accumulated.

Plan of Correction: We will repair the paint on the wall in this class. The play tables will be emptied and cleaned. We will remind staff to monitor the playground equipment.

Standard #: 8VAC20-780-330-B
Description: Based on observation of the preschool playground, resilient surfacing does not comply with the minimum safety standards.
Evidence:
The fall zones at the base of the slides did not have a minimum of six inches of resilient surfacing.

Plan of Correction: We will rake the mulch to ensure the resilient surfacing is maintained. Staff will be reminded to monitor this daily.

Description: Based on observation of the center, food brought from home was not properly labeled.
Evidence:
Lunchboxes brought from home was not labeled with the child's name and date in Classroom #3.

Plan of Correction: We will remind parents to properly label all food brought from home. Staff will monitor the food brought from home and label and date any food that has not been labeled by parents.
