Comments:
An unannounced monitoring inspection was conducted today.
All rooms were in ratio during the inspection. Children were observed playing on the playground, eating snack, washing hands, eating lunch and transitioning to the outside.
Violations were reviewed with the Head of School at the exit interview.
2 staff records were reviewed today. Please send the 3 child records and one staff document requested to me by close of business today.

Description: Based on a review of records, one staff record did not contain documentation of a negative Tuberculosis (TB) screening at least every two years.
Evidence:
Staff B's record contained documentation of a negative TB result dated 2/10/18. (Expired)

Description: Based on staff statement and review of the written allergy lists available, a current list of children's allergies was not maintained in each room or area where children are present.
Evidence:
1. Classroom 7 staff stated they did not have an allergy list in the room.
2. Classroom6's allergy list was dated 8/2023.

Evidence:
During the inspection, 3 child records and one document for a staff record were not available.
