Comments:
An unannounced monitoring inspection was conducted today from 10:15 am-12:40 pm. There were 27 children directly supervised by 5 staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, learning about insects and eating snack. Diapering/ potty routines and hand-washing procedures were also observed. There was an ample supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Based on observation, equipment was not maintained in operable condition.
Evidence: lawn mower push toy had cracked plastic pieces on the playground.

Based upon observation food brought from home was not clearly dated and labeled that identifies the owner.
Evidence: 2 lunches in the toddler classroom were not labeled with the date and the child's name.
Plan of Correction:
I will remind staff to use the tape to label the lunches.
