Comments:
An unannounced renewal inspection was conducted today from 10:30 am-2:00 pm. There were 43 children directly supervised by 8 staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating lunch. Diapering and hand-washing procedures were also observed. Parent pick up, head counts, transitions from outside to classrooms and outside supervision was observed. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Based on record review and staff statement, there wasno annual update available for review of the written injury prevention plan.
Evidence: There was no injury prevention plan completed or could be located for 2023.

Description:
Based on review of records, staff record did not contain documentation of two references prior to employment.
Evidence: Staff A's record contained references , however there was no date available to verify that they were completed prior to employment.

Description:
Based on record review and staff statement, the center did not complete and or/ document monthly evacuation drill. Evidence: There were no evacuation drills completed for the 9/2023, 11/2023, 12/2023.

Description:
Based on observation, when food was brought from home, it was not clearly dated and labeled in a way that identifies owner.
Evidence: Children's House 5- 5 lunches from home were not labeled with name and date.
Plan of Correction:
There is a note to parents on the classroom door to remind them to label lunched. We are trying to make sure they do it.
