Comments:
An unannounced monitoring inspection was conducted today from 10:50 am until 12:30 pm. There were 7 children (22 points) directly supervised by the provider and 2 additional staff. The child care area, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, coloring, and eating lunch. Caregivers were observed engaged in conversation with the children and actively participating in their play. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider did not maintain a record for two assistants which contained all required information.
Evidence:
The records for Staff 1 and 2 did not contain: verification of age, job title, date of employment, ot the name address and telephone number of a person to be notified in an emergency.

Plan of Correction: I have the form and I will have the staff members complete the information.

Description: Based on record review, four of four children's records were incomplete.
Evidence:
1. The records for Children 1-4 did not contain documentation of a health insurance policy number.

Plan of Correction: I will ask the parents to provide the policy numbers.

Standard #: 22VAC40-111-200-A
Description: Based on record review, the provider did not orient the substitute provider and assistant by the end of their first week of assuming job responsibilities.
Evidence:
On the date of inspection (10/24/2019) there was no documentation present in the file for Staff 1 and Staff 2 that they had received an orientation.

Plan of Correction: I will provide and document an orientation.

Standard #: 22VAC40-111-240-B
Description: Based on observation, one piece of equipment present in the family day home has been recalled by the U.S. Consumer Product Safety Commission as being hazardous.
Evidence:
A Fisher Price "Rock 'n Play" sleeper was observed in the child care area. All Fisher Price "Rock n' Play" sleepers were recalled in 2019 as they pose a risk of child death.

Plan of Correction: Disposed of immediately.
