Comments:
An unannounced renewal inspection was conducted today from approximately 9:00 am-10:15 am. At the beginning of the inspection there were 3 children (7 points) directly supervised by the substitute provider. The provider arrived at the family day home at approximately 9:30 am. The child care area, 3 staff records, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely including dramatic play, building with blocks, and dancing. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on record review, the assistants' and substitute providers' records shall contain all required information. Evidence: The records for Staff 1 and Staff 2 did not contain verification of age.

Description:
Based on observation and provider statement, the provider did not record and sign the time of arrivals and departures on each day that the substitute provider works. Evidence: 1. At the beginning of the inspection the substitute provider was observed providing primary care for the children present and the provider was not present. 2. The provider did not have a record of the time of arrivals and departures on each day that the substitute provider works.
