Comments:
An unannounced renewal inspection was conducted today between 11:25am and 12:10pm. The provider does not currently have any children in care. A complete inspection of the physical plant, household member records, menu and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on interview, each adult household member has not completed a sworn disclosure statement. Evidence: Household member #1 turned 18 years old in July 2022. The provider confirmed that the household member has not yet completed a sworn disclosure statement.
Plan of Correction:
They are not currently living at home, but will return after they finish bootcamp. I will have them fill it out when they return.
