Comments:
An unannounced renewal inspection began on January 9, 2020 and ended on January 22, 2020. There were 10 children in care from ages 4 years old to 10 years old with two staff providing care and supervision. The inspector reviewed 5 children's records onsite and all new staff records at another location on January 22, 2020. The center did not have any medications on the premises. This center does not transport children. Children were observed during the following activities: arrival, hygiene practices, snack time, homework time. The inspector also reviewed emergency drill documentation, first aid kits, injury/incident reports, policies and procedures, and emergency supplies. There were discussions about the renewal process and children's records. The inspection took place from approximately 2:50 PM to 4:30 PM. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion, the center failed to ensure that each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all the elements as required by the standards.

Evidence:

1. Records for Children 1-5 did not contain home addresses for each parent who has custody. Staff 2 confirmed that the records did not contain a field to fill in addresses for each parent who has custody.

Plan of Correction: Site Director has corrected all items. In the future, we will ensure we have correct files before submitting to sites.

Standard #: 22VAC40-185-90--A
Description: Based on record review and discussion with staff, the center failed to ensure that a written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include all of the elements as required by the standards.

Evidence:

1. There was no written agreement between the parent and the center in the record for Child 4. Staff 2 verified that Child 4 did not have an agreement between the parent and the center.

Plan of Correction: Site Director corrected this violation on the same day.

Standard #: 22VAC40-185-240-B
Description: Based on record review and discussion with staff, the center failed to ensure by the end of the first day of supervising children, staff shall be provided in writing with the information listed in 22VAC40-185-420 A and all the elements as required by the standards.

Evidence:

1. The Staff Handbook, as described and identified by the School Age Childcare Coordinator, did not contain:

Procedures for supervising a child who may arrive after scheduled classes or activities including field trips have begun;
Procedures to confirm absence of a child when the child is scheduled to arrive from another program or from an agency responsible for transporting the child to the center;
Procedures for identifying where attending children are at all times, including procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center;
2. The School Age Childcare Coordinator verified that these items were not in writing.

Plan of Correction: Childcare Coordinator is creating the staff and parent handbook to incorporate all missing items.

Standard #: 22VAC40-185-420-A
Description: Based on document review and discussion with staff, the center failed to ensure that before the child's first day, parents shall be provided in writing, all elements as required by the standards.

Evidence:

1. The Parent Handbook, as identified by the School Age Childcare Coordinator, did not contain the center's transportation policy, the center's policy regarding application of sunscreen and insect repellent and a policy for reporting suspected child abuse as required by the Code of Virginia.
2. The School Age Childcare Coordinator verified that these items were not in writing.

Plan of Correction: Childcare Coordinator is creating the staff and parent handbook to incorporate all missing items.
