Comments:
An unannounced monitoring inspection was conducted on 10/03/2023 and concluded with an electronic record review on 10/5/2023. There were 7 children, ages 5 years- 9 years, and 2 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. The children were observed during arrival and self-directed activities. A total of 5 children?s records, 2 staff records, and 1 medication were reviewed. The inspector discussed the following with the provider: medication. The inspector arrived for the inspection at 2:45 pm and departed at 3:45 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review and interviews with staff, the center failed to obtain documentation that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence: The record for Child 3 contained a documented first day of attendance of 08/10/2023. There was no documentation of a physical examination in the record for Child 3. Staff 1 confirmed there was no documentation of a physical examination available for review.

Evidence: The record for Child 1 did not contain the following information: phone number of child?s physician and address of two designated people to call in an emergency if a parent cannot be reached.
