A subsidy health and safety (SHSI) inspection was conducted on June 21, 2023 with the center director and staff. There were 52 children present with 6 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Based on staff interview, the provider failed report to the department within 24 hours the serious injury to a child while under the center's supervision. Evidence: On 4/28/2023, Child A suffered a serious injury which required outside medical attention. The incident was not reported to the department until 7/29/2023.
Plan of Correction:
An IROMT was completed online.
