Comments:
An unannounced renewal inspection was conducted on 02/15/2024 from 12:00 p.m. to 3:30 p.m. There were 107 children present, ranging in ages from two months to five years old with 28 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of records and interview, the center failed to obtain documentation of a negative tuberculosis screening for each staff member at the time of employment.
Evidence:
1. Staff #7?s start date was 01/17/2024. The tuberculosis screening on file was dated 01/22/2024.
2. Staff #11 confirmed that Staff #7?s tuberculosis screening was not obtained at the time of employment.

Description: Based on review of records and interview, the center failed to maintain a separate record for each child that contained the required information.
Evidence:
1. Child #3?s record was missing the second emergency contact.
2. Child #5?s record was missing name, address, phone number, place of employment and work phone number for one parent.
3. Child #10?s record was missing annual documentation the child?s information was up-to-date.
4. Staff #11 confirmed the items were missing from the records.
