Comments:
An unannounced monitoring inspection was conducted today from 9:46am-10:30am with the provider on site. There were 11 children with 3 caregivers. A sample of 2 children's files and 4 caregivers's files were reviewed. The children were engaged in the following: toy play, circle time, singing in Spanish and reading books. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Based on review of four caregivers records, the provider did not obtain for each caregiver a current report of tuberculosis screening form every two years. Evidence: 1. Caregiver #4 had a tuberculosis screening form dated 2/02/2021 and had not been updated.
