An unannounced complaint inspection was conducted from 10:12 a.m. until 11:55 p.m. in response to a complaint that was received in this licensing office on 01/27/2020 concerning class ratios, room capacity, and staff illness in the facility. Interviews with staff were held, records were reviewed, and observations made. The preponderance of evidence gathered during the investigation supports the allegations of classroom ratios. An intensive plan of correction meeting has been schedule at the Western Regional Dept. of Social Services for February 11, 2020 at 10:00 a.m. Please contact your inspector at 276-698-9981 with any questions.
Based on observation, record review, and interview, the center failed to maintain ratio in the infant room. Evidence: 1. The inspector interviewed staff #1 who stated the infant room had been out of ratio in the morning due to a staff person for that room calling in sick. 2. Staff #2 verified that the infant room was out of ratio from 8:10 a.m. until 9:08. 3. The inspector viewed the class roster which verified what the staff had reported.

Description:
Based on interview and record review, the center temporarily reassigned children from their regular group casually and repeatedly which disrupted the children's schedule. Evidence: 1. Staff #1 stated that child #1, #2, #3, and #4 were being moved everyday in the afternoon everyday and the required paperwork had not been completed.
