Comments:
An unannounced monitoring inspection was conducted today from approximately 1:20pm-2:40pm. There were 3 children (15 points) in the direct care and supervision of 1 caregivers. A sample of 3 children?s records were reviewed. The children were observed playing with toys, engaged with the provider and napping. Areas of non-compliance are identified in this report. Based on record review, the provider did not obtain a physical examination for each child in attendance:
Evidence: 1. Children B and C did not have a physical examination on file. 2. Provider stated these children began in September/2019

Evidence: 3 of 3 records were missing required information 1. Child A was missing the name, address and phone number of an emergency contact. 2. Children B and C were missing the name, address and phone number of an emergency contact; medical insurance provider and policy number, physicians address and phone number and verification of idneitification.

Based on record review, potentially poisonous materials were not inaccessible to children.
Evidence: Air conditioner, and men's shaving cream was located on top of the cover of the toilet.
