Comments:
An unannounced monitoring inspection was conducted on 11/5/19 from 7:50 am until 12:00 pm at Open Door Baptist Church, Inc. to review the religious exempt requirements. At the time of entrance 13 children were present with 5 staff. The sample size consisted of five children's records and seven staff's records. Children and staff were observed during indoor free play, structured educational and artistic activities, lunch, arrival, hand washing, clean-up, transitions and behavioral guidance. Violations were found during this inspection and are documented on the violation notice.

Standard #: 63.2(17)-1716-A-9
Description: Based on observation and interview, the center failed to be in compliance with all safe sleep guidelines recommended by the American Academy of Pediatrics.

Evidence:

1. There were two cribs in the infant room with toys attached to the sides of the crib.
2. Staff 6 stated that the children that use both of the cribs are under the age of 12 months.

Plan of Correction: The toys will be removed from the cribs. All staff will be advised that the only items allowed in a crib for children under 12 months of age are the baby, crib, fitted sheet and mattress.

Description: Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining cushioning material under playground equipment.

Evidence:

The playground does not have cushioning material under the slide.

Plan of Correction: The children will not use the slide until cushioning material can be added.
