Comments:
An unannounced monitoring inspection was conducted to investigate a self reported incident related to the forbidden action of physical discipline of a child at the center. The inspection was initiated with the director and the local agency on January 29, 2020 beginning at 1:00 PM and ending at 1:53 PM. An exit meeting was conducted on March 3, 2020. One staff record, one child's record, documentation, and video recordings were reviewed. A violation was issued as a result of this monitoring inspection. . Based on a review of video recording and staff interviews, a staff member utilized the forbidden actions of physical punishment and roughly handling of a child at the center. Evidence: Staff A was observed on the video recording from the playground area on January 27, 2020 roughly handling a child by pulling Child A, age 4 years old, off the middle of the slide by his arm. The force of the pull caused the child's body to go over the side of the slide. Child A landed on his feet after going over the edge of the slide. Staff A grabbed the hand of a second child and roughly pulled both children to line up with the rest of the class outside of the door leading to the classroom. Staff A is observed letting go of the second child's hand and hitting child A.The child can be heard crying out immediately after impact and continuing to cry as the class entered the building.
Plan of Correction:
The teacher was terminated immediately after the incident. All staff are being trained on behavior management through Pro Solutions which needs to be completed by March 31, 2020.The staff will also be trained on stress management and management will reiterate that help will be provided during difficult and stressful periods.
