Technical Assistance was offered regarding medication forms.
Comments:
An unannounced non-mandated inspection was conducted from 10:10 am to 11:30 am to assess the day to day operations of the center within the conditional period. There were 42 children present with 7 staff members. The children were observed completing art projects, dancing, and playing outside. 4 staff records and 5 children's records were reviewed. Medications and emergency drill records were also reviewed. Areas of non compliance are listed in the violation notice. Based on record reviews, the center did not obtain completed Sworn Disclosure Statements (SDS) for employees. Evidence: Staff # 1, Staff # 2 and Staff # 3's records did not have current and completed SDS forms.
