An announced mandated initial inspection was conducted at the facility on Monday, August 12, 2019 from approximately 10:00am until 11:15am. The licensing inspector was assisted during the inspection by a member of management and the site director. During the inspection, compliance with some standards could not be determined since the center is not yet in operation. Toys, educational materials, games, arts and crafts materials and other items for the children to use have been purchased. The required postings were reviewed and found to be in compliance. The first aid kit and non-emergency supplies were inspected and found complete. During the inspection, two sample staff records were reviewed. The most current health inspection was dated: 04/18/2019. The most current fire inspection was dated: 10/01/2018. Based on a review of two staff records, observation, and interview on 08/12/2019, the center did not ensure there was one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
Evidence: 1) The licensing inspector did not observe daily health observation training in the record for Staff #1 (DOH: 01/28/2019). Staff #1 was present during today's inspection. 2) The licensing inspector did not observe daily health observation training in the record for Staff #2 (DOH: 10/29/2018). 3) During interview, a member of management reported Staff #1 and Staff #2 do not have current daily health observation training.
Plan of Correction:
