A Subsidy Health and Safety Inspection (SHSI) was conducted on 1/26/2024 from 1:40pm to 3:40pm. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.
Based on record review and interviews it was determined the vendor did not inform the departments representative s soon as practical, but not more that two business days, of an injury to a child while under the vendors supervision when a referral is made for treatment from a medical professional. Evidence 1. On 10/19.2023 a child was injured while in care of the center. The child required outside medical care. 2 On 10/24/2023 a licensing inspector directed, and provided the link to complete, the required "Child Requiring Medical Treatment" form on the departments website. 3.The regional manager agreed to complete the form immediately, however the "Child Requiring Outside Medical Treatment" form was not completed.
