Comments:
Licensing Inspector arrived at Douglas Park Elementary School at 12:00 and departed at 1:30 PM to review staff records. The sample size was two staff records.
Licensing Inspector arrived at the facility at 3:20 and departed at 4:00 PM to conduct a monitoring visit. At the time of the inspection, there were seven school-aged children in care with two staff. Children were observed completing homework, eating snack and playing with legos. The sample size was five children's records.
Based on review of five children's records, it was determined that the facility did not ensure that all required elements are present in each child's record.
Evidence: The records for child #2 and child #4 did not contain addresses for the required emergency contact persons.
Plan of Correction:
Site director will contact the parents to obtain the addresses.

Description:
Based on review of two staff records, it was determined that the facility did not ensure that all required elements were present in each staff member's record.
Evidence: 1. The record for staff #1 was missing one of the two references as to character and reputation to be checked prior to employment and documentation of orientation to include hire date. 2. The record for staff #2 was missing both of the two references as to character and reputation to be checked prior to employment and documentation of orientation to include hire date.
Plan of Correction:
District manager will ensure that all required elements are present in each staff member's record.
