Comments:
Licensing Inspector arrived at 6:15 and departed at 7:10 AM to conduct the site visit for a renewal. At the conclusion of the inspection, there was one child present with two staff. Child was observed playing with legos. There was no medication at the site. The sample size was five children's records. A staff record review was done the same day at the AlphaBEST office. The sample size there was two staff records.
Based on review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.
Evidence: The record for child #1, date of enrollment 1/7/20, did not contain addresses for the two required emergency contact persons.
Plan of Correction:
Site director will ensure that addresses are added to record and will continue to audit records for completion.
