Comments:
An unannounced monitoring inspection was conducted on 07/24/2023. There were 37 children, ages 3 months-5 years, and 10 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. A total of 5 children?s records, 5 staff records and 2 medications were reviewed. The children were observed in during nap time and having an afternoon snack. Infants were observed being fed and sleeping on demand. The inspector arrived for the inspection at 1:00 pm and departed at 3:10 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review and discussion with staff, the center failed to obtain a copy of the criminal record check and a sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 3 contained documentation that Staff 3 had resided outside of the state of Virginia within the preceding five years. The record for Staff 3 contained a documented hire date of 08/16/2021. There was not a criminal record check from all of the states in which the individual had resided in the preceding five years available for review during the 07/25/2023 inspection.

Description:
Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment or prior to coming into contact with children and that the documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence: 1. The record for Staff 1 contained a documented hire date of 06/14/023. The record for Staff 1 did not contain documentation of a negative tuberculosis screening; the tuberculosis screening was not obtained at the time of employment or prior to coming into contact with children. 2. The record for Staff 5 contained a documented hire date of 01/09/2023. The tuberculosis screening in the record for Staff 5 was dated 09/09/2022; the tuberculosis screening had not been completed within the last 30 calendar days of the date of employment.

Evidence: 1. The record for Child 1 did not contain documentation of viewing proof of the child?s identity and age. 2. The record for Child 2 did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached (the address was incomplete for the 2nd designated person listed)

Evidence: The record for Staff 3 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the orientation training.
