Comments:
A monitoring inspection was conducted on 3/20/2023 with the regional director and center staff. There were 62 children present, ranging in ages from 5 years to12 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Staff records were shared electronically on 3/21/2023. A total of 6 child records and 3 staff records were reviewed. The children were making paper kites, playing outdoors, and playing a group game in the gym.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of six children's records, the center did not obtain all required information for two children's records. Evidence: 1. The record of child # 3 did not have the name and phone number of the child's physician. 2. The record of child # 5 did not have the name and phone number of the child's physician or the name and phone number of each parent's employment.
Plan of Correction:
Center staff will request the information from the parents.
