Description:
Based on observation, the provider failed to ensure that all children's hands were washed as required. Evidence: after a diaper change, the provider put the infant on the floor to play without washing the child's hands.

Description:
Based on observation, the provider failed to dispose of soiled diapers and wipes in such a way that the caregiver's hand nor diaper touches the exterior surface of the storage system during disposal. Evidence: after a diaper change, the provider first placed the soiled diaper on a shelf above the changing table. After placing the infant on the floor, the provider moved the soiled diaper to a foot operated leakproof storage system however she did not use her foot to open it. The provider's hand and the diaper touched the top of the storage system during disposal.
Plan of Correction:
Moved the foot operated trash can next to the changing table.
