The Licensing Inspector conducted a self-reported unannounced other inspection on 07/14/2022. The inspector was on site from 12:00pm until 1:30pm. Staff were interviewed, and documentation of the incident was obtained. There were no violations from the self-report, but non-related violations were cited to the program.

Based on record review and interview, the center failed to obtain a copy of the person?s completed sworn statement or affirmation before employment.
Evidence: Staff #1 which was hired on 07/26/2021, had no documentation of the Sworn Statement on file. There was no documentation present during inspection on 07/14/2022.
