Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A (monitoring inspection was initiated on 10/22/2020 and concluded on 10/28/2020. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 1 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff / household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on a review of records and interview, the provider did not obtain documentation of a physical examination by the child's physician for every child within 30 days of attendance. Evidence: The provider was asked to provide specific enrollment documentation for a sampling of 2 children in care. The submitted paperwork for child #1, whose date of enrollment was documented as 1/15/2020, did not contain results of a physical examination. The physical examination was requested again by the inspector but was not submitted.
Plan of Correction:
The licensee will locate or obtain the results of physical examination for child #1 and place it in the child's record upon receipt, within 30 days.
