Comments:
A subsidy health and safety (SHSI) inspection was completed on 08/23/2023. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.
Based on a review of records and interview, the provider did not annually attend at least 16 hours of training, to include the department's health and safety update course. Evidence: The last documentation of having taken the department's annual health and safety training was dated 6/15/2022. The provider confirmed that this was the last time she completed the department's annual health and safety training .
Plan of Correction:
The provider sent documentation of having completed the department's annual health and safety training on 9/5/2023.
