Comments:
An unannounced monitoring inspection was conducted on 8/29/22 from 9:45am - 10:45am. At the time of the inspection there were six children in care, ages three months old to four years old (14 points) with two caregivers present. Records were reviewed for five children and two caregivers and one household members. Children were observed participating in various activities. There was no medication at the family day home currently. Emergency plan and emergency supplies were reviewed. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed with the provider during the exit interview.
Based on a review of five child records, it was determined that the provider did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence: 1. The record for child #1 did not contain documentation of an immunization record. 2. The record for child #2 did not contain documentation of an immunization record. 3. Caregiver #2 confirmed that child #1 and child #2 did not haven an immunization record available for viewing during the inspection.
Plan of Correction:
The provider responded: The parents of each child will be asked to provide a current immunization record. All new children will not start unless there have a current immunization record.
