A renewal inspection was conducted on July 28, 2023 between the hours of 11:15 am through 12:15 pm. There were three children present (9 pts), ages 6 months to 2 years old, with one caregiver. The inspector reviewed children's records, one caregiver record, and one household member record. Areas of non compliance are identified on the violation notice. Based on record review, children's records did not contain required information. Evidence: 1. Child #1's record (Start date-05/21/2023) did not contain documentation of physical examination results.
Plan of Correction:
The provider will get a copy of the physical examination from the parents.
