Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 9/22/2020 and concluded on 9/23/2020. The director was contacted by telephone to initiate the inspection. There were 29 children present and eight staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children's records and five staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of children's records the center failed to obtain all the required information in a timely manner. 1. One of three records reviewed did not have a proof of identity or birth certificate on file. Child record # 2 began care on 8/13/19 and there is no proof of birth in the record.
Plan of Correction:
The director will review all the children's files to ensure that all the required information is in the file. Contact will be made with the parent/custodian to obtain this information.

Description:
Based on review of staff records the facility failed to enusre that any employee of a licensed facility who is involved in the day-to-day operations or who is alone with, in control of, or supervising one or more children have a fingerprint based criminal history record check prio to first day of employment. Evidence: 1. Staff # 1, hire date 8/1/2020; staff #2 hire date 6/1/2020; staff #3 hire date 8/31/2020; and staff #4 hire date 8/3/2020 do not have a finger print background check prior to the first day of employment. 2. Staff # 1 hire date 8/1/2020, staff 4 hire date 8/3/2020 do not have a central registry search on file before 30 days of employment.
Plan of Correction:
Staff # 1, 2, 4 had appointments for fingerprint based background checks on 8/14/2020, 8/18/202 and 9/24/2020 respectively, however, the results have not been received by the center. Staff # 3 will have her appointment scheduled. The program director will contact OBI and request information regarding the fingerprint background checks. Staff #1 & 4 have sent off for their central registry checks however, the center has not received these yet. The program director will contact OBI to request information regarding these searches.
