Technical Assistance:
Technical assistance today included children's records, outdoor play procedures, the 800 Poison Control number, director's qualifications, age limits for the license.

Comments:
An unannounced monitoring inspection took place today between 1:15 - 2:45 PM. There were six children present with two staff. I observed storytelling and discussion before nap; and an older child who was kept occupied with learning tasks and quiet conversation. Standard #: 22VAC40-185-60-A
Description: Based on a review of records today, not all children's records have all required information.
Evidence:
The enrollment form did not include the question asking parents if any special accommodation is required to meet the needs of their child (this is required).
Additionally, there was incomplete emergency contact information for Child # 1, 3 and 4; and incomplete work information for the parents of Child # 1.

Plan of Correction: We will add the information about special accommodations and make sure parents complete all questions, with no blank spaces.

Standard #: 22VAC40-185-240-D-5
Description: Based on a review of records today, there were no staff present today with current Daily Health Observation training as described in 240 D6.
Evidence:
Although staff did have documentation of on-line hours in Penn State Preservice training for health and safety, it did not meet the criteria as outlined in 240 D6 which includes information regarding the Virginia Dept of Health and OSHA training.

Plan of Correction: We will take the Daily Health Observation Training as required.
