Technical Assistance:
Technical assistance today included record keeping, required postings, emergency evacuation documentations, supervision, access to emergency information.
Comments:
An unannounced renewal inspection was conducted today from 10 am - 12:40 pm. There were 7 preschool-aged children present at the beginning of the inspection with one staff. At 10:10, the second staff returned from walking a child to the school bus, and another child arrived. I observed children painting, working with Playdough, listening to stories in circle time, building with blocks, counting and preparing for lunch and nap. Children were engaged and busy throughout. Based on a review of children's files, not all children had documentation of up to date immunizations before beginning care. Evidence Child # 1, who began the program December 2019, did not have proof of immunizations as required before beginning care.
Plan of Correction:
The parent has been reminded. We will remind again and future students will not begin without a record of shots or a letter.

Description:
Based on a review of records today, one child did not have a physical exam on file within 30 days of beginning. Evidence: Child # 1 began the program 12/2019 but does not have documentation of a physical as required

We will talk to the parent today.

22VAC40-185-60-A
Description:
Based on a review of records today, not all required information was documented in all children's files. Evidence: There was not a complete name for the two emergency contacts for Child #6, and incomplete or missing addresses for emergency contacts for Child # 2, 3 and 5.
Plan of Correction:
We will go through the files and make sure all emergency contact information is complete.

Description:
Based on a review of children's records today, there was no signed parent agreement form for one child in care. Evidence: The parent completed a different enrollment form which did not include the required agreements.
Plan of Correction:
The parent will complete a new form.

Description:
Based on observation during today's inspection, there was an area not maintained in a safe condition. Evidence:The throw rug near the entrance was observed to be on top of the floor without being anchored, posing a slipping hazard.

We will remove the rugs.

Based on observation at the beginning of the inspection, there were not two adults present as required when children were present.. Evidence: At 10:00 am, there were 7 children present with one staff. The second staff arrived at 10:10 am.

I was taking a child to the bus for kindergarten, We will make sure there are always two adults present.

Description:
Based on observation today, not all items required for diapering were present. Evidence: There was no step-on trash container as required for diaper disposal..

We will buy one.
