An announced initial inspection was conducted on Tuesday, September 10, 2019 from 10:05am to approximately 12:52pm. During the inspection, compliance with some standards could not be determined since the facility is not yet in operation. Compliance with these standards will be determined during unannounced visits once the program is in operation. The center is equipped with age and stage appropriate materials in all classrooms and playgrounds. All areas of the facility including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. Transportation will be provided. Three buses and transportation documents were reviewed. Medication will be administered by the center's licensed practical nurse (LPN), registered nurse (RN), or by staff that possess current medication administration training (MAT). During the inspection, the following items were reviewed and discussed; parent handbook policies, staff policies and procedures, emergency preparedness plan, emergency supplies, storage of medication, storage of cleaning supplies, menus, daily attendance sheets, Infant feeding schedule, and daily sheets. Two staff records were reviewed during the inspection. Though, some standards of care could not be determined during the initial study, the center is in substantial compliance with the standards that could be determined, and will be recommended for a six-month conditional license at this time.

Based on observation and interview on 09/10/2019, the center did not ensure all areas and equipment of the center were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence: 1) During the inspection, the licensing inspector observed sharp and splintering wood on areas at the bottom of the playground fence on the hill towards the far right end of the playground. 2) During interview, a member of management reported several trees had been recently cleared in that area and the center is in the process of repairing the fence.
Plan of Correction:
Per the Center: "Repairs to the fence began prior to the pre-license inspection and are scheduled to be fully completed by 09/20/2019, which is prior to the scheduled date the new license will take effect. Site Administration will continuously monitor building and grounds and schedule with maintenance to complete any repairs that may arise in the future."

Description:
Based on observation on 09/10/2019, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.
Evidence: 1) During the inspection of the playground, the licensing inspector measured the mulch under and around the swings near the preschool building and composite climbing structure located near The Village's playground. The mulch measured approximately 3 to 5 inches in depth. The equipment requires at least six inches of resilient surfacing.
Plan of Correction:
Per the Center: "Play areas were dug out and additional mulch was added to create a depth of 9 inches, which is 3 inches more than the required 6 inches. This was completed on 09/16/2019, which is prior to the scheduled date the new license will take effect. Site Administration will continuously monitor mulch depth to ensure ongoing compliance."
